This is an 8.5” x 11" form that features a centrally located integrated card measuring 3.375” x 2.125”. The card is die-cut from the same stock material as the form, located at the bottom of the sheet. It has a 2 mil lamination on both the front and back. The back liner will remain attached to the form after the card is removed.

The integrated card can be utilized for various purposes, including membership cards, club cards, security passes, business cards, ID cards, insurance cards, health care cards, and marketing applications.
